Shiawassee Health & Wellness (SHW) is helping individuals who visit our agency get their health records electronically 24/7 through the Community Electronic Health Record (CEHR) system.
CEHR is a web-based service. It offers a safe, private way to get your personal healthcare information as well as other related wellness services. It also is a way to communicate with health workers.
Warning: Do not use CEHR to contact your provider in an emergency. If this is an emergency, contact your provider directly by phone or call 911. CEHR should only be used to communicate non-urgent and non-critical questions to your care provider, such as information about your medications or test results or follow up questions from a recent visit.
You will need to talk to your case holder to have your progress notes, assessments, and Person-Centered Plan entered into your CEHR account. Next, you will need to log into your CEHR account. Once you are logged in, you will be able to send a message to your case holder and see your account information, such as:
Next appointment date and time
Payment information
History of diagnosis, allergies, and medications
Latest lab and test results
Progress notes
Person-Centered Plan
Other health information, including healthy eating, exercise, and other wellness related information.
