EDUCATION:
Bachelor’s degree in social work, clinical psychology, or counseling from an accredited university.
CERTIFICATION/LICENSURE:
Bachelor's or Master’s degree in social work or a related field from an accredited university or college. Ability to be credentialed as a QMHP.
EXPERIENCE:
One year's experience working with individuals with chronic mental illness, intellectual disability and substance use disorders. 

Two years’ experience in a human services agency and knowledge about psychiatric disorders, intellectual disabilities, and substance use disorders.
SUMMARY OF RESPONSIBILITY:
Case Coordinator to provide on-going assessment, person centered planning, coordination of care, linkage of agency and inter-agency resources and advocacy services. The Case Coordinator is the primary provider when an individual needs multiple services. The Case Coordinator is responsible for facilitating the Person-Centered Planning process (PCP) so that regulatory/Medicaid compliance is assured, and the PCP reflects consumer strengths, desires, dreams, needs and goals. The Case Coordinator advocates for individual needs by linking, advocating, and monitoring the utilization of community-based services and resources. The Case Coordinator is a member of the Adult Mental Health Case Coordination team and reports to the Supervisor of Adult Mental Health Case Coordination.
