Home and Community Based Services

Shiawassee Health & Wellness provides Home and Community Based Services (HCBS) to qualified Medicaid beneficiaries in their own home or their community rather than in institutional settings.    The Centers for Medicare and Medicaid Services have specific guidelines for agencies providing home and community based services.
