What Is CEHR?

Shiawassee Health & Wellness (SHW) is helping individuals who visit our agency get their health records electronically 24/7 through the Community Electronic Health Record (CEHR) system.

CEHR is a web-based service. It offers a safe, private way to get your personal healthcare information as well as other related wellness services. It also is a way to communicate with health workers.

How Do I Get My Electronic Health Records Anytime?

You will need to talk to your case holder to have your progress notes, assessments, and Person-Centered Plan entered into your CEHR account. Next, you will need to log into your CEHR account. Once you are logged in, you will be able to send a message to your case holder and see your account information, such as:

• Net appointment date and time
• Payment information
• History of diagnosis, allergies, and medications
• Latest lab and test results
• Progress notes
• Person-Centered Plan
• Other health information, including healthy eating, exercise, and other wellness related information.
